Who Is Eligible to Participate?
We have taken the lead in the area of preventive dental treatment by offering this program. As one of our patients, you and any members of your immediate family (household) are eligible to participate for a fixed annual fee. Use a separate application form for each enrolling member.

When Do Benefits Start?
Membership begins as soon as your application and payment are processed. For the term of your agreement, you will receive dental services at the reduced, preferred rates exclusive to program participants.
